1.   This form consists of five parts

• Part 1 Entry Form
• Part 2 Application for Seeding & Declaration
• Part 3 Personal Information for Media
• Part 4 Medical & Emergency Contact Details
• Indemnity & Declaration form are attached for ease of printing

2.   Please fill in the form clearly and legibly.
3.   ll sections of the form should be completed. If a section is not relevant please fill in “NOT APPLICABLE“.
4.   The completed form should be submitted to the Secretary of the Meet at

CHECKLIST

Please remember that the originals of all enclosures must be shown for validation to the Secretary of the Meet and the photocopies must be self certified.
Please ATTACH the following with the completed entry form.

The entry is considered complete ONLY when it is submitted with ALL the following:

• Entry Fees in Full with a copy of the receipt 
• Security Deposit for Tracking Device 
• Completed Entry Form 
• Entrant, First Driver & Second Driver to submit Indemnity Form separately on Rs.50/ non-judicial stamp paper duly notarised. 
• 3 Passport Size photographs each of Both Drivers with names & blood groups on the back. 
• Letter of Authority from registered owner of vehicle, (If other than Entrant) permitting use of vehicle. If the owner is other than an individual then the authority letter must be on the owner’s official letterhead. The person signing the letter must give his full name and designation and the letter must bear the stamp / seal of the owner. 

PHOTOCOPY OF THE FOLLOWING.

• Registration Book of Vehicle
• Normal insurance of the vehicle
• Driving License
• Competition Licence of FMSCI: ( If not available then a completed form of licence application with the relevant documents & fees)
• Blood Group Report of First & Second Drivers from any Medical Laboratory or Hospital
• Third party Rally insurance of the vehicle
• High Risk Personal Accident insurance with hospitalisation benefits for Rs.2,00,000/- or above

IMPORTANT

•   ENTRY FORM IS NOT CONSIDERED COMPLETE UNTIL THE ENTIRE FORM IS COMPLETED IN ITS ENTIRETY AND ALL ATTACHMENTS / VERIFICATIONS HAVE BEEN SUBMITTED AND ENDORSED.
•   THE DATE OF COMPLETION OF THE ENTRY FORM AS ABOVE WILL CONSTITUTE THE DATE OF ENTRY AND ENTRY FEES WILL BE CHARGED ACCORDINGLY
